Parkway Christian Academy ~ Application Checklist

Request a packet of information or download (everything in blue) from the web.
- Fill out a student application
- Read/Download Handbook
- Sign Parent-School Agreement along with other packet information
- Return information to PCA at least 2 days prior to interview along with:
  - Student application
  - A copy of a current report card and a transcript (High School)
  - Most recent standardized testing information
  - IEP, 504 Plan, etc. if applicable (any special education documentation)
  - Disciplinary Report from current school
  - Physical Education Consent Form
    Photo Release Form
    Transportation Permission Form
    Procedures for Arbitration
    Field Trip Release Form
    Birth Certificate (copy)
    Immunization Record (copy)
  - Pre-K and Kindergarten students will need to submit a copy of an entrance physical form from their doctor
Schedule interview with the senior school administrator. The following must be present for the interview:
- Both parents (or parent with sole custody if applicable)
- All children seeking acceptance

Submit the following paperwork at the end of the interview:
- Student Record Release Form
- Uniform Order Form
Submit registration fee with paperwork to secure enrollment after acceptance

Meet/ Speak with Financial Administrator, Cheryl Turner, to sign tuition payment schedule or sign before leaving interview
